This conference chaired by Dr Sharon Chadwick, Medical Director, Hospice of St Francis, Berkhamsted, Macmillan Consultant in Palliative Medicine, West Hertfordshire Hospitals NHS Trust will focus on improving end of life care for people with Cardiovascular Disease and Heart Failure through a person centred approach. Patients with cardiovascular conditions, including Heart Failure have been disproportionately impacted by the Covid-19 pandemic. The conference will reflect on how we can improve care for people at the end of life, embed high quality supportive care pathways, improve communication about end of life care & advance care planning, and understand progression of Heart Failure: predicting and recognising last days of life and managing uncertainty. The conference will also discuss best practice in symptom control.
Heart and circulatory diseases cause around a quarter of all deaths in the UK; that’s more than 160,000 deaths each year, or 460 each day – that’s one death every three minutes.
In total it’s estimated that more than 900,000 people in the UK have heart failure, there are around 690,000 people in the UK are on their GP’s heart failure register, Around 200,000 new diagnoses of heart failure every year in the UK.
